A Guide to Clearing the Infection for Good

It starts as a small white or yellow spot under the tip of your toenail. You ignore it, assuming it will grow out. But months later, your nail is thickened, brittle, and discolored. You try covering it with polish or using over-the-counter creams, but nothing seems to work.

Onychomycosis (fungal nail infection) is one of the most common (and most frustrating) conditions we treat. Because the fungus lives under the hard nail plate, it is incredibly difficult to reach and destroy. What is Happening Under the Nail?

Fungal nails are caused by microscopic organisms called dermatophytes. These fungi thrive in warm, dark, moist environments—exactly like the inside of your shoes.

  • The Invasion: The fungus enters through tiny cracks in your nail or the surrounding skin (often starting as Athlete’s Foot).
  • The “Shield”: Once inside, the fungus feeds on the keratin in your nail. The nail plate itself acts as a shield, protecting the fungus from topical creams you apply on the surface.

Signs It’s Fungus (Not Just Trauma)

How do you know it isn’t just a bruise from running? Look for these signs:

  • Thickening: The nail becomes difficult to trim.
  • Discoloration: Yellow, brown, or white streaks.
  • Brittleness: The nail crumbles or splits easily.
  • Debris: A buildup of dark material under the nail plate (subungual debris).

It’s worth noting that a fungal nail is contagious, and it will compromise nearby skin. That, in turn, may increase the likelihood of a plantar wart infection.

Why Drugstore Treatments Fail

Many patients spend hundreds of dollars on pharmacy lacquers before seeing us.

  • The Problem: Most OTC treatments cannot penetrate the thick nail plate to reach the nail bed where the infection lives. They essentially treat the “roof” while the infection continues in the “basement.”

Professional Treatment Options

To clear the infection, we have to bypass the barrier.

  1. Prescription Topicals: Unlike drugstore versions, these are formulated to penetrate the nail plate. They work best for mild, early-stage infections.
  2. Oral Antifungals: These pills work from the inside out, delivering the medication through your bloodstream to the nail bed. While highly effective, they require monitoring for liver function.
  3. Debridement: We can thin the thickened nail significantly, removing the infected layers and allowing topical medications to penetrate deeper.

IMPORTANT NOTE FOR PATIENTS MANAGING DIABETES:

Weakened immunity and compromised circulation make those with diabetes more vulnerable to fungal nail infections! Even worse, the presence of peripheral neuropathy makes detection harder, leading to additional issues like non-healing wounds.

Prevention: Protect Your “After” Photo

Fungus is contagious. To prevent reinfection:

  • Sterilize Your Shoes: Use antifungal sprays or UV sanitizers in your footwear.
  • Wear Shower Shoes: Never go barefoot in gym showers or pool decks.
  • Rotate Shoes: Give your shoes 24 hours to dry out completely between wears.

Don’t Wait for Sandal Season
