A Guide to Plantar Fasciitis

When you wake up, your first step hurts. It’s like a hot stone or a nail on your heel. You limp to the bathroom, and slowly, the sensation fades to a dull ache. But the moment you sit down for lunch and stand up again? The pain is back.

This “First Step Pain” is the classic symptom of Plantar Fasciitis, the most common cause of heel pain in adults. While many people assume it will go away on its own, ignoring it can lead to chronic inflammation and months of unnecessary aches. In this blog, Valley Podiatry Associates will help you prevent that! Read on to learn what is happening inside your foot…and how we fix it.

What is Plantar Fasciitis?

The plantar fascia is a thick band of tissue (ligament) that connects your heel bone to your toes. It acts as the bowstring for your arch.

  • The Inflammation: When this band is irritated or micro-torn, it becomes inflamed.
  • The “Morning Sting”: When you sleep, your feet relax and point downward, allowing the fascia to shorten and tighten.
    • When you stand up, you are suddenly forcing that tight tissue to stretch under your full body weight. That sharp pain is the tissue tearing again.

Why Me? (The Root Causes)

It isn’t just bad luck. It is usually a structural issue.

  1. Faulty Mechanics: People with flat feet or very high arches are naturally prone to this. If your arch collapses or is too rigid, the fascia takes on excessive load.
  2. Work Environment: Standing for long hours on hard, flat surfaces (concrete/tile) without supportive shoes breaks down the tissue.
  3. Obesity: Extra weight puts exponential stress on the arch with every step.

Diagnosis: Is it Really Plantar Fasciitis?

Heel pain isn’t always simple. Before we treat it, we must rule out other mimics:

We perform a comprehensive exam and may use X-rays or imaging to ensure we are treating the right problem.

(Note: We often see Heel Spurs on X-rays, but they are usually the result of the pulling, not the cause of the pain!)

Fixing the Foundation (Non-Surgical Treatment)

Surgery is rarely needed.

We successfully treat the majority of cases with a tiered, conservative approach, starting with:

Tier 1: At-Home Defense

  • Stretching: Targeted calf stretches loosen the “chain” pulling on your heel.
  • Ice: 20 minutes several times a day (use a towel barrier!) to calm inflammation.
  • No Barefoot Walking: Never walk without support. It strains the fascia immediately.

Tier 2: Medical Intervention

If home remedies fail after a few weeks, we step up the protocol:

  • Orthotic Devices: This is the gold standard. Custom orthotics correct the underlying structural abnormality (like flat feet). They don’t just cushion; they align.
  • Night Splints: A device worn while sleeping to keep the fascia stretched gently, eliminating that morning pain.
  • Injection Therapy: Corticosteroid injections can break the cycle of inflammation for immediate relief.
  • EPAT Shockwave TherapyA non-invasive option available in our office, this treatment uses high energy acoustic pressure waves to stimulate the body’s natural healing response.
